Variety of Voices

The needs assessment is a comprehensive survey that provides the opportunity for a variety of voices to share their level of need, interest, and desire on four pillar topics. The Needs Assessment helps identify and prioritize a range of school-based programs and services to support student learning and development.

The survey offered through the community school grant comprises two sections: one addressing general family needs and interests, and the other tailored to individual student interests and needs. Completion of the student section is mandatory for all students at the 8 campuses within the grant program to ensure that your input contributes to the overall understanding received by campus site coordinators.

The Full-Service Community School team aims to meet, welcome, and assist families in survey completion because your input is crucial for identifying and prioritizing ways to support students and families. From October to December, the team will distribute the survey to various groups, and you can also expect to find them at the Parent-Teacher Conferences in October.

The Full-Service Community School team will be present in either the media center or STEM Labs at your child's campus during parent-teacher conferences to assist parents in survey completion. The survey is confidential, but it does require an authentication process, which enables us to streamline the survey by omitting information already known, such as the school attended and grade level, and so on.

Key Steps:

Site Coordinators will be provided with a list of Student IDs and Dates of Birth (DOB) to assist parents in survey completion.

1. Authenticate the survey response using your child's school ID and DOB.
2. Fill out the Family Section.
3. Complete the Student Section.
4. If you have another student, provide their Student ID and DOB.
